Your First Visit

Before Your First Appointment

When you make your first appointment with IVFMD, you will receive an email with your unique username and password to access your Patient Portal. Please login to your Patient Portal and fill out the required forms as soon as possible to secure your appointment. 

To prepare for your first appointment, we ask that you complete your new patient forms and medical release form at least 2 days prior to your appointment. If you cannot access your medical records before your appointment, please bring them with you.

If your insurance requires you to be enrolled with a fertility benefit, this must be completed prior to your appointment in order to initiate any authorization for your fertility coverage. (Examples of payers requiring enrollment are Maven, Progyny, WINFertility).

What to Bring to Your First Appointment 

Please arrive 15 minutes before your scheduled appointment and bring the following items: 

  • Insurance card and referral, if required. (Note all BCBS members with ID beginning with VMY require a referral from your Primary Care Physician)
  • Photo ID
  • Insurance co-payment, co-insurance, deductible or self-payment (cash not accepted)
  • Any vouchers or discount cards received from your referring physician

What You Will Achieve During the First Visit

This is our opportunity to learn about your story. We’ll review your medical history, listen to your family-building goals, and discuss the testing and treatment options that seem best suited for your situation.  

Your physician will also conduct a physical examination and collect urine and blood samples to try and understand what may be causing your fertility issues. This is a great time to ask questions and discuss any concerns you may have.

What Happens After

After your initial appointment, your designated Patient Care Coordinator will contact you within 1-2 business days to go over the next steps. They will do their best to set up a convenient time to speak with you concerning any testing you will require and assist with scheduling these diagnostic appointments. If you have not spoken to your PCC within 3 business days, please check your messages within the Prelude Connect App. We’ll have an open line of communication to ensure you understand the process and are prepared for each step. 

Financials

Upon completing your initial consult, you will receive a folder containing general IVFMD information, a communication guide, our diagnostic workup checklist, laboratory (blood) contacts, your personal insurance snapshot (if applicable), our basic pricing sheet as well as various financing options (should they be required) so that you can prepare financially as you and your physician decide on a treatment plan.

Financial Follow Up

At IVFMD, we know having accurate financial information is a key factor in the journey you are embarking on. You will receive a financial breakdown of the diagnostic portion, based on your personal insurance or self-pay status within 1 – 2 business days of your new patient consultation via our Prelude Connect App messaging and one of our dedicated financial counselors will contact you within 3 business days to ensure you have the opportunity to review your coverage and address any additional financial questions you have.
