We want to optimize your chances of family-building success from the start. Your journey at IVFMD begins with a 30-45 minute new patient consultation, where we’ll learn more about you and your goals to create a fertility care plan together.
Understanding your medical history is essential to making our first visit as productive as possible. Please complete your new patient forms two days prior to your scheduled visit and sign the release form allowing us to access your medical records from other providers.
This is our opportunity to learn about your story. We’ll review your medical history, listen to your family-building goals, and discuss the testing and treatment options that seem best suited for your situation. You are free to ask questions and share any relevant information with us.
